At Care Crafters, our Customized Living Services provide ongoing support for seniors and adults with disabilities who live in a home based setting and need help with daily living activities. Customized Living includes support with personal care, meals, medication reminders, household routines, safety supervision, and daily structure. These services are designed for individuals who want to remain in their own home while receiving consistent support that helps them live safely, comfortably, and with dignity. What is Customized Living Services ?
Do I Qualify?
To qualify for Customized Living Services at CareCrafters, you must meet the following criteria:
HCBS Waiver Enrollment
You must be enrolled in Minnesota’s Medical Assistance (MA) program and have a waiver that covers Customized Living services.
Age Requirement
You must be 18 years old or older.
Need for Daily Living Support
You must be assessed and need help with everyday activities due to a disability, health condition, or age related needs. Support may include assistance with:
Personal care
Meals and nutrition
Medication reminders
Household routines
Safety awareness and supervision
