Our Customized Living Services help individuals remain in a comfortable residential environment while receiving consistent, personalized support that fits their routine and level of need.

Our team provides dependable support with personal care, meals, medication reminders, household routines, safety supervision, and daily structure. Every service is tailored to the individual so they can live with greater comfort, stability, and independence in a setting that feels like home.

Eligibility

Do I Qualify?

To qualify for Customized Living Services at Care Crafters, you must meet the following criteria:

HCBS Waiver Enrollment
You must be enrolled in Minnesota’s Medical Assistance program and have a waiver that covers Customized Living services.
Age Requirement
You must be 55 years old or older.
Need for Daily Living Support
You must be assessed and need help with everyday activities due to a disability, health condition, or age related needs.
Support may include assistance with:
  • Personal care
  • Meals and nutrition
  • Medication reminders
  • Household routines
  • Safety awareness and supervision
  • Daily structure and support
