Services We Offer

Our Home Healthcare services can include several levels of support depending on the person’s needs, such as:

  • Home care nursing
  • Skilled nursing visits

Examples may include medication support, wound care, chronic condition monitoring, bathing and dressing assistance, transfers, fall prevention, meal help, and other home based support.

Independent Living Services are 245D basic support services designed to help people live more independently in their home and community.

Depending on the person’s authorization and needs, services may include:

  • 24 hour emergency assistance
  • Night supervision
  • Homemaking
  • Community living supports
  • Individualized home supports
  • Respite
  • Transitional services

Customized Living is for seniors and adults with disabilities who need more regular support than they can realistically manage on their own, but who still want a setting that feels more personal and home like.

Support may include personal care, meals, medication reminders, safety supervision, daily structure, and assistance with everyday routines.

Yes. Care Crafters offers transition focused services through Relocation Service Coordination Targeted Case Management and Moving Home Minnesota for eligible individuals moving from qualified institutional settings back into the community.

These services help organize the transition path, coordinate pre move tasks, align community supports, and support a more stable move when the person qualifies.

Eligibility and Funding

Eligibility depends on the type of service, the person’s needs, and the funding source.

  • Customized Living generally requires Medical Assistance, an eligible waiver, and an assessed need for that level of support
  • Independent Living Services generally require waiver funding and service authorization
  • Home Healthcare may involve Medicaid or private pay, depending on the service
  • RSC and Moving Home Minnesota depend on institutional status, program pathway, and other DHS criteria

Yes. Some Care Crafters services are available through Medicaid or waiver funding when the person is eligible and authorized.

This often applies to customized living, 245D independent living supports, and certain transition based services.

That does not always mean there are no options, but it does change which services may be available right away.

Some Home Healthcare and private pay services may still be possible. If the person is being discharged from an institutional setting, transition pathways like Moving Home Minnesota may also depend on separate eligibility rules. Yes. Private pay options may be available for some services, especially within Home Healthcare and related home based support.

Referrals and Getting Started

Not always. Individuals and families are welcome to contact Care Crafters directly.

Some services do require formal referrals, case management involvement, or service authorizations, but if you are not sure where the person falls, you can still reach out first and we can help clarify the next step.

We usually start by learning more about the person’s current situation, what kind of help is needed, who is involved, and what funding source may apply.

From there, we can help identify the best fit, explain whether a referral or authorization is needed, and talk through what the start of services may look like.

It depends on the service. Some people can move faster than others, especially if the funding, authorization, staffing, and documentation are already in place.

If the person needs waiver funded support, institutional transition services, or a residential placement, the timeline may depend on outside approvals and coordination steps. We can usually give better guidance once we know the situation.

It helps to know:

  • What kind of help the person seems to need most
  • Where the person is currently living
  • Whether there is Medicaid, waiver funding, private pay, or another funding source involved
  • Whether there is already a case manager, care coordinator, discharge planner, guardian, or family contact involved
  • Whether the need is ongoing, urgent, or tied to a discharge or move

RSC and Moving Home Minnesota

Relocation Service Coordination Targeted Case Management is a transition focused Medical Assistance case management service for eligible people preparing to move from an institutional setting into the community.

It can involve assessment, planning, referral, coordination, and monitoring activities tied to the person’s relocation goals.

Moving Home Minnesota is the state’s version of the federal Money Follows the Person demonstration. It supports eligible people moving from qualified institutions into qualified community residences through pre transition and post transition services.

On the Care Crafters side, that may include transition coordination, comprehensive community support services, and transitional services within the Moving Home Minnesota pathway.

Yes. If you are a family member, discharge planner, case manager, guardian, or care coordinator and you are not sure which pathway fits, Care Crafters can review the situation and help identify the more appropriate direction.

No. These are separate pathways, and a person cannot receive both at the same time.

Depending on the person’s eligibility and authorization, the Moving Home Minnesota pathway may include:

  • Transition coordination before the move
  • Comprehensive community support services after the move
  • Transitional services tied to approved one time start up needs

Final availability depends on program rules and the person’s situation.
